A nurse in United Kingdom accidentally killed her patient after giving him a wrong blood last 2014. The patient, Mr Huseyin, undergone a heart bypass and the operation was done successfully.
He was then transferred to the recovery unit. Ledesma gave him a blood transfusion even without further incident. Overnight, the patient was then again given another blood transfusion without incident.
Throughout Ledesma’s shift, there were several concerns raised against the health condition of Mr. Huseyin. He was gradually losing fluid through chest drain fitted during the operation. The doctor decided to give him another blood transfusion in the morning.
Ledesma was the one who gave the blood through HemoSafe, a blood vending machine which uses a specific reference number obtained by the all patients.
Only authorized personnel were given the chance to operate this machine and Ledesma had that authorization. Ledesma then wrote the wrong reference number in her hand thinking that it was the right one. There was also a patient in the ward having similar surname as Mr. Huseyin and Ledesma took his details instead.

Another process when dispensing blood was neglected by Ledesma. The blood bag did not contain the same name as her patient but still continued the process thinking that Mr. Huseyin have another name.
To confirm that the blood was actually for the right patient, Ledesma asked him about his date of birth but instead of looking at the blood bag, he was looking at his wristband. Ledesma noted that the patient number is not matching so the issue was raised to her senior colleague.
Ledesma checked the computer and entered the incorrect details and so the blood transfucion was carried out. But instead of giving a blood group O, she gave the wrong blood group which is AB.
The patient was instantly sent for surgery after the wrong blood transfusion. About an hour later, Ledesma confessed that she gave the wrong blood. The patient died that evening.
According to the post-mortem examination, the death was caused by the wrong unit of blood given to the patient.
Police officers were called and Ledesma will face her sentence by February.
